 1937年深秋，為躲避戰亂，中國醫生成賢（秦漢 飾）攜帶懷有身孕的日本妻子理惠子（早乙女愛 飾）和孩子從上海逃到南京老家，他沒想到南京也差不多快被侵華日軍夷為廢墟。被告知可以進入歐美列強勢力保護的安全區避難時，成賢認為自己與難民身份不同沒做理會，投靠了老朋友繼續行醫。以小學教師劉天琴（劉若英 飾）為代表的同胞的悲劇令他震驚憤怒，但他認為自己首要任務不是反抗暴敵而是盡職做好一名醫生。同年冬天12月13日，南京全面淪陷，日軍開始在南京瘋狂燒殺淫掠，成賢終于意識到日軍的殘酷麻木，讓妻子先行一步去安全區躲避，自己則留下來繼續救死扶傷，他不知道的是，安全區早已名存實亡，而要反抗時，他發現已無可能。
